This form is one of the methods for submitting internal reports as defined by the Act of June 14, 2024, on Whistleblower Protection (Journal of Laws 2024, item 928, hereinafter referred to as the “Whistleblower Protection Act”) at Biuro Brokerskie OMEGA Beata Galewska, Piotr Matwijow, Broker Ubezpieczeniowy – Spółka Partnerska (hereinafter referred to as Biuro Brokerskie OMEGA Sp. P.). Reports can also be submitted directly via email at zgloszenia@omegabroker.pl, by phone at (58) 669-23-31, or verbally at the headquarters of Biuro Brokerskie OMEGA Sp. P. located at ul. Żuławska 27, 81-576 Gdynia. Biuro Brokerskie OMEGA Sp. P. will review the report within thirty days from the date of receipt. We would also like to inform you of the possibility of submitting an external report without first filing an internal report, in accordance with Article 30 of the Whistleblower Protection Act.
