In compliance with the information obligation referred to in Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016, on the protection of natural persons concerning the processing of personal data and on the free movement of such data, repealing Directive 95/46/EC (hereinafter: the Regulation), we inform you that:

  • The administrator of your personal data is OMEGA Brokerage Office Beata Galewska, Piotr Matwijow – Insurance Broker – Professional Partnership, headquartered in Gdynia (hereinafter referred to as OMEGA Brokerage Office Sp. P.), ul. Żuławska 27, 81-576 Gdynia, phone: +48 58 669 23 21, email: rodo[at]omegabroker.pl.
  • Your personal data is processed in the following scope: name, surname, PESEL number, residential address, email address, and phone number.
  • OMEGA Brokerage Office Sp. P. [Professional Partnership] processes your personal data for the following purposes:
    • Providing insurance brokerage services on your behalf, including concluding or facilitating the conclusion of insurance contracts, performing preparatory actions for concluding insurance contracts, and participating in the management and execution of insurance contracts, including claims handling (brokerage activities) – legal basis: Article 6(1)(b) of the Regulation;
    • Marketing and promotion of products and services offered by OMEGA Brokerage Office Sp. P. – legal basis: Article 6(1)(a) of the Regulation,
    • Internal administrative purposes of OMEGA Brokerage Office Sp. P., including the preparation of analyses and statistics – legal basis: Article 6(1)(f) of the Regulation;
    • Fulfilling legal obligations incumbent on OMEGA Brokerage Office Sp. P. in connection with conducting insurance brokerage activities – legal basis: Article 6(1)(c) of the Regulation.
  • In connection with the processing of data for the purposes specified in point 4, your personal data will be shared with other recipients or categories of recipients of personal data. The recipients of your personal data may include, in particular:
    • Entities processing your personal data on behalf of OMEGA Brokerage Office Sp. P. based on a data processing agreement (so-called data processors);
    • The Polish Financial Supervision Authority (KNF).
  • Your personal data will be provided to insurance companies for the purpose and to the extent necessary to perform the brokerage activities mentioned in point 1 above.
  • Your personal data will not be transferred to countries outside the European Union or to other data recipients beyond those specified in point 7 above.
  • The personal data you provide will be processed for the period necessary to achieve the purposes of processing specified in point 4 above, namely:
    • For the provision of insurance brokerage services, for the duration of the power of attorney granted by you, and after that period, for the time and scope required by law or to safeguard potential claims. If you have consented to the processing of data after the revocation or expiration of the power of attorney, your data will be processed until this consent is withdrawn;
    • For marketing and promotion of products and services offered by OMEGA Brokerage Office Sp. P., until you withdraw your consent for such processing;
    • For internal administrative purposes of OMEGA Brokerage Office Sp. P., until the legitimate interests of OMEGA Brokerage Office Sp. P. forming the basis of this processing are fulfilled, or until you object to such processing.
  • In connection with the processing of your personal data by OMEGA Brokerage Office Sp. P. [Professional Partnership], you are entitled to the following rights:
    • The right to access data under Article 15 of the Regulation;
    • The right to rectify data under Article 16 of the Regulation;
    • The right to delete data under Article 17 of the Regulation;
    • The right to restrict data processing under Article 18 of the Regulation;
    • The right to object to data processing under Article 21 of the Regulation;
    • The right to data portability under Article 20 of the Regulation.
    You can exercise these rights at the headquarters of OMEGA Brokerage Office Sp. P., by correspondence, via electronic communication, or by phone.
  • In cases where the processing of your data is based on Article 6(1)(a) of the Regulation, i.e., consent to the processing of personal data, you have the right to withdraw your consent at any time without affecting the lawfulness of the processing carried out based on consent before its withdrawal.
  • If you believe that the processing of your personal data by OMEGA Brokerage Office Sp. P. violates the provisions of the Regulation, you have the right to file a complaint with the supervisory authority (i.e., until May 25, 2018, the General Inspector for Personal Data Protection, and after that date, its successor—the President of the Office for Personal Data Protection). Filing a complaint is free of charge.
  • To the extent that the processing of your personal data is necessary for the provision of insurance brokerage services mentioned in point 4(a) above, providing your data is a condition for the performance of these services. Providing your data is voluntary; however, failure to provide this data will result in the inability of OMEGA Brokerage Office Sp. P. to provide the services in question.
  • Your personal data is not processed automatically and is not subject to profiling.
